Before starting the grant application, you must register as a vendor with the State of Mississippi Department of Finance and Administration. If you are already a vendor, you may proceed to the grant application below.

Register as a Grantee and choose CRM-ORG as the type of Grantee. You will be required to enter a phone number and email address. We recommend this be the information for an individual who is responsible for the finances at the school.
Grantees who have completed the registration process and have received a User ID and Password must submit a W-9 for their account. You can mail a valid W-9 to:

If you had rather email your W-9, you can send it to: ofmmagic@dfa.ms.gov. If you choose to email your W-9, be sure to include your school's name and User ID in the body of the email.
Any registration issues you run into will have to be resolved by DFA, not MAIS or MDA as they do not have access to help with supplier registration issues.
Point of Emphasis: The mailing address that is used on the vendor application must match the mailing address on the MDA application below.
